This is the information you will need to have prepared when you apply for SSI or SSDI benefits.

- Your Social Security number and the Social Security numbers and birthdays of your spouse and children.
- A third-party contact person, usually a friend or a family member who can give Social Security information about your daily activities. You will need this person's address and phone number.
- Your work history for the past fifteen years. You do not need the employer's name or address, but you do need the dates you worked, your job title, your job duties, and the physical requirements, including how long you sat, stood, walked, and lifted at each job.
- All doctors that you have seen for all impairments that prevent you from working. You will need their names, addresses, their specialty, what services they provided and the first and last dates you were treated. If your condition has lasted significantly before you stopped working, you only need to list the doctors seen in the year before you stopped working through the present.
- Your current medications and their side effects.
- The dates of your last medical tests including blood tests, x-rays, MRIs, etc.
- If you are applying for SSI, a list of your assets including bank accounts, stocks, bonds, payroll slips, burial fund records, and general information about the things you own.
